Case Manager – SHINES Program

Join Laraway Youth and Family Services as we expand services to students with ASD and/or other intensive needs! 

The SHINES (Specialized Health and Instruction to Nurture Exceptional Students) Case Manager is responsible for creating and maintaining alignment between the student’s families/caregivers and educational partners. In collaboration with the SHINES Clinician, they gather relevant information from a wide variety of team members to craft, monitor, and update Individual Treatment Plans. This position works closely with the SHINES Special Educator to coordinate student IEP services and community learning opportunities. The SHINES Case Manager is stationed in person on campus. It is common for this position to take lead in small group daily living skill, regulation, and recreational activities. As the primary supervisor of Intensive Needs Behavior Interventionists (INBIs), they are responsible for observation, feedback, and documentation.

Essential Duties

  • Initiate and maintain communication with families/caregivers, service providers from partner agencies, and sending schools regarding treatment offered to the student and services available to the family.
  • In concert with, and under the direction of, the Licensed Independent Clinical Social Worker (LICSW) update Biopsychosocials and develop/update Individualized Plans of Care (IPCs).
  • Monitor and document progress toward IPC goals and maintain corresponding digital documentation.
  • Inform, model, and supervise implementation of a variety of instructional and intervention strategies as outlined in the assigned student’s Positive Behavior Support Plans (PBSPs) and individualized programming protocols.
  • Maintain up-to-date knowledge of each assigned student’s Individualized Education Plan (IEP).
  • In the event of restraint or seclusion, facilitate compliance of Rule 4500 timeframes and associated expectations of communication and documentation, as well as necessary LYFS associated documentation.
  • Be on time for the workday

Therapeutic and Behavior Services

  • The BCBA may not be in attendance at all team meetings. In advance of meetings, determine their attendance and communicate with them to prepare for presentation of analysis of the behavioral data and trends.
  • Prepare updates and provide LICSW with progress overviews during scheduled meetings and as requested or determined appropriate.
  • Provide training, guidance, and support for INBIs, including training new hire INBIs and internal cross training.
  • Collaborate with SHINES Coordinator and Educators to prepare for and implement program activities and instructional opportunities.
  • Actively center student success by professionally fielding needs, observations, and behavioral changes as presented by INBIs and other team members.
  • Oversee, support, and provide feedback to INBIs regarding data collection and reporting required for progress monitoring and billing (E.g. ABC sheets, daily data, work samples, etc.).
  • Coach, observe, and provide structured feedback to INBI’s regarding their implementation of IEP accommodations, PBSP interventions, and individualized programing instructional strategies.
  • Facilitate and supervise small group periods of skill development and practice (i.e: play-based blocks, social skills groups, sensory and regulation practice, daily living skills).
  • Manage the physical safety of students and others by utilizing antecedent management, de-escalation strategies, and physical interventions as trained and certified.
  • Prepare for, respond to, and facilitate crisis debriefs as necessary.
  • Support development and maintenance of a positive learning environment by adhering to trauma informed approaches; model therapeutic relationships.
  • Fill in as Intensive Needs Behavior Interventionist (INBI) or Pod Lead as needed
  • Provide transportation to and from community activities.
  • Other duties as assigned

Administrative

  • Support INBIs to complete Mandated Reporter notifications to DCF.
  • Communicate with the SHINES Coordinator to identify substitute coverage needs, as necessary.
  • Team with Pod Lead to support their supervision of student sub-plans, as necessary.
  • Work in concert with Pod Lead and Coordinator to ensure staff training logs are up to date.
  • Collaborate with Pod Lead to effectively and efficiently monitor daily home/school journal communications between INBIs and caregivers/family.
  • Follow supervision directives and provide weekly supervision to INBIs.
  • Complete all agency, billing, and program paperwork accurately, within the outlined timeframes and in accordance with federal and state mandates.
  • Actively participate in variety of meetings as assigned (i.e.: Staff meeting, supervision, treatment team meetings, etc.).
  • Actively engage in ongoing professional development such as agency, program, team or individual training.

Qualifications and Education Requirements

  • Bachelor’s degree in human services or related field, however, bachelor’s degree nearing completion and relevant experience with a strong staff development plan may be considered.
  • Two years’ experience with human services delivery serving children, youth and families, preferred.

Special Position Requirements

  • This position is a Mandated Reporter, as such, is required by law to report suspected abuse or neglect to authorities.
  • Must adhere to confidentiality expectations in all forms of communication.
  • Clearance of Criminal Background Check, maintenance required.
  • Valid driver’s license, excellent driving record, and daily access to a safe, reliable, insured vehicle, required.
  • Must be able to work effectively within the structure of the school, keeping Laraway ethics and conduct expectations in mind.
  • Ability to complete and maintain Handle with Care verbal and physical certifications; Trainer certification preferred.
  • Ability to pass First Aid & CPR training and maintain certification, Lifeguard certification preferred.
  • Basic knowledge and ability to use word processing programs as well as standard communication tools such as telephone systems and e-mail.

Work/Physical Requirements

  • Given the at-risk population served, the position requires ongoing training and preparedness in the execution of physical restraint techniques. This may require running to catch a child.
  • The “Handle with Care” program specifies that individuals with back and neck limitations are likely to be hampered in the execution of the protocols and greatly increases risk of harm to self or student.
  • Ability to engage with youth in a variety of activities ranging from art, music, cooking and physical activities such as walking, hiking, swimming, and other outdoor sports.
  • Ability to lift up to 50 lbs.

Pay Range: $23.20 – $29.88 per hour – depending on education, certifications/licenses, and relevant experience
